Legal names (including any name changes)
Email addresses
Phone numbers

Mailing addresses
If you have recently moved, changed your contact information, or updated your name,

we need your current details for our records.

You can download the update form fill it out, save it and email it to us.

​and we’ll e-mail a form to you.
